On the north east coast, midwife Sheena looks after a woman who recently had a miscarriage, and is concerned about her pregnancy this time around. Pam, who works at Belford Hospital in the town of Fort William below Ben Nevis, hopes to deliver Fiona's second baby there, just as she delivered her first child. Fiona's first baby was premature, and Pam is concerned that might happen again. Meanwhile, Karen and her husband Stuart face a problem as neither of them drive and could struggle to get to the hospital when Karen goes into labour.

Claire wants a local delivery in Belford Hospital rather than have to visit Inverness to be induced, while in the village of Ellon, Vicky has very high blood pressure and a condition that is dangerous for both mother and baby.
